<p style="text-indent: 0px; margin: 0px;"> The Glasgow Coma Scale was not originally intended to be a ubiquitous neurological scoring system. There is evidence to demonstrate the GSC is confusing, unreliable, and unnecessarily complex, and its manner of common clinical use is statistically unsound. Therefore we should consider using Simplified Motor Scales (SMS) that provide the same information, are easier to remember, are statistically derived and externally valid (p. 428)</p>

<p style="text-indent: 0px; margin: 0px;">This article is an editorial, so it is not primary research. The article references a number of other primary sources of evidence including recently published peer-reviewed literature from well respected journals.  Referenced studies indicate the GSC has poor inter-rater reliability, it is not consistently remembered, there are different versions (usually outdated 12-point evaluations), it is inconsistent which prevents prognostic value (p. 428  par 1)</p>

<h3 style="text-indent: 0px; margin: 0px;">Quotable:</h3>
<p style="text-indent: 0px; margin: 0px;">"It is time to abandon the Glasgow Coma Scale (GCS). As discussed below, this ubiquitous neurologic scoring system is confusing, unreliable, and unnecessarily complex, and its manner of common clinical use is statistically unsound." p. 427</p>
<p>"Some traditionalists will no doubt object to the blasphemy of "dumbing down" the GCS, but why tolerate pointless complexity?" p. 428</p>
<p>"The GCS is intellectually appealing to health care providers in that it creates apparent order out of disorder. It ambitiously tackles the enormous complexity of human neurologic response and organizes (oversimplifies) it into a tangible, appealing yardstick that cannot fail to impress with its seeming accuracy and precision. We want to believe that medicine can be this objective, and thus we tolerate the delusion." p. 429</p>
<h3 style="text-indent: 0px; margin: 0px;">So What?</h3>
<p style="text-indent: 0px; margin: 0px;"> </p>
<p>This article was fascinating to me for two reasons. First, as a student I remember trying very hard to memorize the GCS and found it difficult, but thought I was the problem. Second, working in a simulation lab now I find it interesting to see how students do tend to want black and white answers. It is fascinating to step back and see the dialogue that happens in the literature as evidence evolves and researchers have to push for change, not an easy thing to do. As an educator I wonder how we make this apparent to students, since we have to mark them on something and they want very defined and specific answers, yet their knowledge will hopefully and should always be changing (read: evolving) with new evidence.</p>

<p><a href="http://www.von.ca/en/about/ceo.aspx">Chief Executive Officer</a> -<a href="http://www.von.ca/"> Victoria of Nurses</a> (VON)</p>
<p>Beginning her career as a clinical nurse Judith Shamian earned her way to the Chief Executive office at the Victoria Order of Nurses. Along the way she has learned many things and feels strongly about the importance of mentoring nurses in hopes of inspiring future nursing leaders. In our conversation how we discuss the inspiration for beginning her research and her advice for aspiring nursing students and nurses. Among her many words of wisdom Judith recommends nurses find their a purpose they feel strongly about and a passion to learn about how organizations work. Learning to make decisions based on available knowledge, learn from mistakes and constantly reflect on your work is another critical lesson she passes on to future leaders.</p>

<p>Peter Goossens, a Dutch mental health nurse and researcher, and I sit down to discuss his research that helps bi-polar patients recognize on coming periods of mania and depression. We met at the Sigma Theta Tau 40th Biennial and discussed his fascinating research with mental health patients and their families. He has not only conducted excellent research he is allow a great ambassador for the profession of nursing, working to ensure the are able to fully able to support their patients and the nurses perspective is represented in national consortiums.</p>

<p>I really must apologize to Chris about this podcast. We had a great conversation in January last year (2009). We met at the <a href="http://www.cna-aiic.ca/cna/default_e.aspx" target="_blank">Canadian Nursing Association</a> - <a href="http://www.cna-nurses.ca/CNA/documents/pdf/publications/NLC_Brochure_2009_e.pdf" target="_blank">Nursing Leadership Conference</a>. She had a very interesting idea about how to engage with nursing staff. She works in public health who have their fingers on the pulse of what familes, clients and communities partners need form nursing leadership. Using the World Cafe approach she was able to engage and pull ideas from key stakeholders of her nursing practice. Below are the pictures of what resulted from this approach.</p>

<p>An experienced and passionate physician in the field of infection control, Dr Michael Gardam is now working with the Agency for Health Protection and Promotion on provincial issues of protection and control. After hearing him give a talk at Princess Margaret I knew I needed to speak with him. In this talk he shares major insights into how Ontario began to public reporting, what they are reporting, how that is perceived and the many issues around hand washing in hospitals.</p>

<p>Dr Mary Jane Esplen has an extensive career in clinical oncology practice that has evolved over the years to include research as major part of her role. Her ability to drive research strongly tied to clinical practice and how she managed to stay in both academia and practice is part of what we talk about. More recently she has been appointed <a href="http://www.desouzanurse.ca/about_staff.shtml" target="_blank">Director of the de Souza Institute</a> for nursing oncology knowledge transfer for the province, and is planning to take this international. We discuss how her career evolved, why she loves psychosocial oncology and how others might learn from her experience.</p>

In January, 2004 she received the Atkinson Charitable Foundation’s Economic Justice Award which will last for up to three years. During this period of time she is “housed” at the Sherbourne Health Centre.</p>
